BLEEDING FROM RESPIRATORY TRACT - ASPECTS OF INTERVENTIONAL AND RADIOLOGICAL DIAGNOSIS

Abstract

Objective. Evaluate the distinguishing features of radiologic and interventional diagnostic procedures at the presence of bleeding from lower respiratory tract, performed at LUHS KK. Materials and methods. Retrospective analysis of data form 26 patients, evaluated at LUHS KK (departments of pulmonology and emergency care) for suspected acute bleeding from lower respiratory tract. The sequel and efficiency of routine radiologic (roentgenography – X-ray, computed tomography – CT) and interventional (fibrobronchoscopy – FBS) procedures were evaluated. The sequel and diagnostic accuracy of radiologic and interventional procedures were evaluated by the bleeding intensity at the divided groups. Bleeding was set to be massive when the blood loss in 24 hours was > 100ml, non-massive < 100ml. Results. Massive bleeding was diagnosed for 26.9 per cent, non-massive 73.1 per cent. All the patients underwent the chest X-ray, FBS – 21 (80.8 per cent), CT – 19 (73.1 per cent). Evaluating the cases of non-massive bleed found: CT was performed for 14 (73.7 per cent), of those the reason or localization of bleeding was found in 12 (85.7 per cent) cases. Meanwhile with the FBS, which was performed for 15 patients (78.9 per cent), the source for bleeding was diagnosed in 5 (33.3 per cent) of them. In the group of massive bleeding from lungs, using the data from CT the source of bleeding was diagnosed in 4 cases of 5 (80 per cent), where the data from FBS was accurate in 5 cases from 6 (83.3 per cent). Treatment using FBS was performed at 5 cases from 6 (83.3 per cent). Conclusions. After evaluation of potentialities of different interventional and radiological diagnostic methods, possible causes of hemoptysis, consider the economical aspects, when massive bleeding is not presented, it is recommended to perform CT scan.
